Service Navigator for the Pioneer Valley
Job Type: Full-time
Schedule: Mon-Fri only
Location: must live in or near the service area

The Pioneer Valley Service Navigator will administer the UCP’s Adult Family Care (AFC) contracts, MRC MassAbility (ABI/MFP and Supported Living) contracts, statewide Head Injury Program contracts and DDS Agency with Choice contracts. Responsibilities include navigation, documentation, weekly and monthly check-ins in compliance with the respective contracts’ regulations and guidelines. The goal of this role is to provide support, information and resources and navigation of the systems of care to families and caregivers of individuals with disabilities.
Service Navigator Responsibilities
Coordinate and monitor members’ needs, including referrals and emergency assistance
Develop, implement, and review individualized care plans
Conduct regular on-site visits with each member bi-monthly (Level I)/monthly (Level II)
Conduct evaluations and training for caregivers
Participate in developing and reviewing Supported Living Plans of Care
Make referrals for additional services as needed
Provide direction and training for support staff
Ensure compliance with DDS regulations and guidelines. This includes creation of budgets, attending necessary ISP meetings, HCSIS documentation, supervision of staff, regular check ins with the family
Utilize ISP goals to address individual needs
Maintain communication with families and members
Maintain up to date records and documentation
Provide timely assistance and respond to emergencies
Participate in investigations and provide direct service as needed
Offer transportation and administer CPR/First Aid when necessary
Service Navigator Requirements
Bachelor’s degree AND
Social Work license from the Massachusetts Board of Registration in Social Work in good standing with two years of experience working with elder or disabled adults OR
Minimum of two years of clinical experience working the relevant population
With Mass Health approval: Associate degree with 5 years experience working with the relevant population
Knowledge of local community resources and community-based providers and programming.
Ability to maintain an Adult/Pediatric CPR and First Aid certification.
Ability to set up and maintain a home office.
Comfortable with remote applications (ex. Zoom, teams, Microsoft Office, etc.)
Ability to maintain confidentiality and privacy and show cultural competence.
A valid driver’s license, proof of insurance and the availability of a private vehicle
Negative Mantoux TB test or a clear chest x-ray.
Satisfactory CORI, SORI, DPPC, National Fingerprint background checks.
