Description Summary: The Family Nurse Practitioner/Physician Assistant insures delivery of comprehensive health care in accordance with the philosophy, standard of practice, objectives and policies of The Institute for Family Health. The Nurse Practitioner is responsible for diagnosis and treatment of common and chronic illness focusing on health promotion and disease prevention activities including patient education and counseling in the Institute of Family Health.
REQUIREM ENTS: - Clinical/Technical/Service
- Demonstrates the ability to perform clinical/ technical/ service/ administrative tasks:
Health Assessment, Treatment, and Maintenance
Demonstrates competent level of technical skills.
Conducts health assessment interviews.
Performs complete physical assessment.
Orders and interprets diagnostic tests.
Develops plan of care based on the diagnosis and individual client needs.
Prescribes medications and treatments, as appropriate.
Works collaboratively with the health care team and makes referrals to other disciplines, as indicated.
Records, reports, and interpret the client’s responses to plan of care.
Teaches, counsels and assists individuals and families to assume responsibility for prevention of illness and the promotion, maintenance and restoration of health.
Collaborates with other members of the team in the provision of regular health screening and other health promotion measures.
Identifies available community resources helpful to clients, refers and assists clients in utilizing such resources.
Makes home/hospital visits to clients and/ or takes calls, as appropriate.
Demonstrates the knowledge and skills necessary to provide care, based on physical, psychological, educational, safety and related criteria, appropriate to the age of the patients served in assigned area.
Provide med-phone coverage, as directed by supervisor.
Provides feedback to Nurse Manager, Practice Administrator and/or Medical Director regarding policies and practices affecting the performance of the practice and/or their individual performance.

- Determines own professional needs through ongoing self-assessment, peer and care team feedback and evaluation of role performance and assumes responsibility for meeting those needs.

- Bachelor’s degree and Master’s degree in Nursing
- Graduate of an accredited Nurse Practitioner program
- Bilingual, Spanish preferred.
Experiences And/Or Skills Required - Strong FNP clinical experience.
- Basic computer skills.
- Ability to solve problems
- Leadership ability.
- Effective organizational and interpersonal skills
Licenses/Certifications Required - Current NY State Registered Nurse License and Registration.
- Nurse Practitioner Certification and Registration.
- Current Practice Agreement.
- Certified or eligible for certification by recognized specialty organization.
- Current BCLS, Infection Control, Child Abuse
