Job Summary: This will be a full-time, direct placement position. Licensed Practical Nurses (LPNs) play a critical role in the healthcare system by providing essential nursing care under the supervision of Registered Nurses (RNs) and physicians. As an LPN, you will be responsible for delivering compassionate, patient-centered care in various healthcare settings, including nursing homes, outpatient care facilities, and long-term care environments. Your duties will include monitoring patients' health, administering medications, assisting with wound care, and providing personal care to ensure patients’ comfort and well-being. In collaboration with RNs and doctors, you will contribute to the development and implementation of patient care plans, helping to achieve positive health outcomes. The demand for LPNs has risen significantly due to staffing shortages, making this role both vital and rewarding.
Monitor Patient Health: Regularly check and record patients' vital signs, including temperature, blood pressure, pulse, and respiratory rate. Observe patients for changes in condition, behavior, or appetite, and promptly report any concerns to the RN or physician.
Medication Administration: Safely administer prescribed medications, including oral medications, injections, and intravenous fluids, following proper protocols and documenting all administered doses.
Wound Care and Dressing Changes: Provide basic wound care, including cleaning and dressing wounds, applying bandages, and monitoring for signs of infection or complications. Ensure that wound care is carried out in accordance with the prescribed treatment plan.
Personal Care Assistance: Assist patients with activities of daily living (ADLs), such as bathing, dressing, grooming, eating, and toileting. Promote patient independence and dignity while providing personal care.
Patient Education: Provide patients and their families with education on their health conditions, treatments, and medications. Offer guidance on maintaining health and preventing complications.
Care Plan Implementation: Collaborate with RNs, doctors, and other healthcare professionals to develop and implement individualized patient care plans. Participate in care conferences and contribute to decision-making processes regarding patient care.
Documentation: Accurately document all aspects of patient care, including observations, treatments, medication administration, and patient responses, in the electronic health record (EHR) system.
Patient Safety: Adhere to safety protocols and infection control procedures to ensure a safe and clean environment for patients, staff, and visitors.
Emotional Support: Provide emotional support and comfort to patients and their families, helping them navigate the challenges of illness, recovery, and long-term care.
Licensure: Must hold an active and unencumbered LPN license in the state of Indiana.
Education: Completion of an accredited LPN program.
Certification: Current Basic Life Support (BLS) certification is required.
Experience: Previous experience in a clinical setting, particularly in long-term care or outpatient facilities, is preferred but not required. New graduates are encouraged to apply.
