Here’s what you’ll be doing:
- Assists with the development and implementation of multiple strategies, initiatives and activities for individuals, families and population groups that promote health and prevent disease within the FCP Community.
- Assists in the development, implementation and evaluation of strategies that address established community health improvement priorities in partnership with Tribal and Local partners.
- Designs and conducts health educational programs for the FCP Community, including community health screenings, immunization clinics, and educational events.
- Serves as Public Health Accreditation Coordinator and ensures the Community Health Department is meeting all requirements to maintain Public Health Accreditation.
- Develops and promotes and/or markets health education materials and information; delivers targeted, culturally appropriate information.
- Gathers, organizes, and maintains data for program monitoring, evaluation, and improvement.
- Assists in the development and coordination of community and agency partnerships, initiatives, programs, and events Promotes the community’s understanding of, and advocacy for, policies and activities that will improve the community’s health.
- Develops and maintains various departmental plans including Community Health Improvement, Quality Improvement, and Community Health Assessments.
- Maintains and promotes the Tribal Wellness program and facilitates Tribal Wellvation meetings.
- Implements health education and youth programing in partnerships with other Forest County Potawatomi entities; completes Event Evaluations for Health Education and Health Promotion events and programs held by the department.
- Assists in the department’s grant efforts which may include trainings, conferences, and meetings.
- May also use nursing process to provide home and office visits to Tribal Members with an emphasis on health promotion, disease prevention, assessment of services needed, interventions, education, and screenings across all age groups.
- Identifies the appropriate resources necessary to provide solutions to barriers encountered by a client in obtaining recommended services as indicated by medical diagnosis, physical condition, or social need.
- Obtains patient histories, provides skilled nursing services, takes vital signs, oxygen levels and blood glucose readings, delivers medications, medication box set-up, medical supplies and medical equipment as needed. Documents all client specific data into the official client medical records using an identified E.H.R., WIR or other data collection systems.
Here’s what you’ll need to be successful:
- Registered Nurse with a Bachelors’ Degree in Nursing, Public Health, Health Education, Health Promotion or other related field.
- Current Wisconsin’s State Nursing License in good standing.
- CPR and Basic Life Support Certified.
- Must have and maintain a valid driver’s license and be legible for coverage under the Tribe’s insurance.
- Must successfully pass all applicable background checks and drug screens
- FCP complies with all CDC vaccine and testing recommendations for healthcare professionals. Proof of immunizations is required and immunizations/testing provided upon hire.
Preferred Qualifications:
- Three (3) years of experience in Public Health or Community based setting preferred.
- Grant Writing experience preferred.
